He presses the first button (the icepick gets driven into his eye), but he fails to press the other one in time and, as a result, his limbs are torn from his body.

The mute man then opens his mouth and screams; blood pours from his mouth.
A pregnant woman miscarries her child after a man opens a door and accidentally slams it into her stomach. Blood is shown dripping down her legs.
